From A Hospital
The hospital staff will supply a Patient Review Instrument (PRI) and Screen
two documents required by New York State that describe the particular care and services needed by the applicant. These forms must be supplied along with our completed Application for Admission.

From The Community
The same forms described above must also be supplied for applicants who are not hospitalized. In this case, the PRI, Screen and Home Assessment forms are completed by a qualified assessor or agency. Our Admissions Department will gladly assist you.

Payment Plans
Medicare, Medicaid, most private insurance plans, managed care and private pay are all accepted at Workmen’s Circle MultiCare Center. Our staff members are available to
assist you in financial matters and to discuss individual financial questions in depth.
