From Rockland County & Upstate New York
Tappan Zee Bridge to route 287 eastbound. Take 287 eastbound to exit for Sprain Brook Parkway southbound which becomes Bronx River Parkway. Exit to Cross County Parkway eastbound to Hutchinson River Parkway southbound. Exit at exit 7 and then follow directions as above from Hutchinson River Parkway.
From Queens & Long Island
Whitestone or Throggs Neck Bridge to New England Thruway (Route 95) north. Proceed to exit 12 (Baychester Avenue) which exits on left. Turn left at traffic light and go 2 blocks to Grace Avenue. Turn left on Grace Avenue to Workmen’s Circle MultiCare Center.
From George Washington Bridge
Follow signs to Cross Bronx Expressway eastbound to Route 95 Northbound (New England Thruway). Take Route 95 to exit 12 (Baycheser Avenue) which exits on the left. Turn left at traffic light and go 2 blocks to Grace Avenue. Turn left on Grace Avenue to Workmen’s Circle MultiCare Center.
From Manhattan, West Side
West Side Highway (Route 9) northbound to Cross Bronx Expressway eastbound. Take Cross Bronx Expressway to the New England Thruway (Route 95) northbound. Then follow directions above from George Washington Bridge.

East River Drive northbound to the Bruckner Expressway eastbound.

New York Bus Service Express Bus which stops on Madison Avenue at 23rd Street, 36th Street, 44th Street, 52nd Street and Third Avenue at 87th Street. Take the Express Bus to the stop for Bay Plaza Shopping Center in Co-Op City and transfer to the #28 or #26 bus. Get off at Donizatti Place. Walk across the overpass to Burke Avenue and continue to Grace Avenue. Turn left onto Grace Avenue to Workmen’s Circle MultiCare Center

From Main Street in Flushing, take the QBX-1 Bus to Co-op City. Get off at Donizatti Place. Walk across the overpass to Burke Avenue and continue to Grace Avenue. Turn left onto Grace Avenue to Workment’s Circle MultiCare Center.
